The purpose for collecting the information will be to process referrals to community services provided by CityCare. The mandatory data fields will also assist CityCare in collecting data about patients and/or clients in which it is commissioned and/or legally obliged to collect.

You are agreeing that any information contained in this referral form will be handled by CityCare staff within the Single Point of Access Team for the purposes of processing the referral. Thereafter the information contained within the referral will be shared appropriately with the relevant CityCare services/teams for consideration by the clinical team. All information provided in the referral will also be accessible by your G.P and any decision made will also be communicated with your G.P accordingly.

All information collected via the online referral form will be kept confidential and secure.
